The one I see violated most often is when an agent fails to get a buyers agency signed then goes on to act like they are representing the buyer, creating an implied agency, while they are officially working for the seller.

I believe the definition of fiduciary can not be broken down into 1 specific action - but always act in the best interest of the client carries the basic intent of the term.
