I wouldn't and I hate those surveys. Our showing service can be set up to automatically send a feedback request to showing agents, which I do. And I prefer to write a response than take a survery.

I often receive calls from agents who got my information from a supra key. 

But if my clients are interested, I would be calling them, right?

 It is a great idea, but it is hard enough to get buyer agents to lock the door and put the key back in the lockbox. My showing service has a tool to send out automatic surveys and maybe 75% come back, i always wonder if they are telling the truth though,.

I would not and this is why.

When I am showing homes for sale, I DO schedule them tightly and I arrive at the time I indicated. (30 minutes plus travel time.) However, I do create adapter mechanisms in case the buyer quickly decides, "No way, let's get out of here."

Your plan has merit, however, I will need a reward for risking falling into a potential trap. This could be a service provided to you by a mortgage broker or lender or other service provider that will enroll me on their list. We all remember that 'INFO Lines monitored by the mortgage broker.  

A 10, 15, 20 question feedback form.....you better have a FREE cup of good java or $$$. Otherwise I won't take the risk. How much is it worth to YOU? Your seller?

However, what works best is when the buyers agent calls YOU to schedule the showing and YOU advise the agent YOU will call them for feedback, the success rate goes WAY UP.  Yes, I am one of those agents who will NEVER fill out an emailed feedback form regardless of it's origin.

I don't believe that I would take the time to do this while with clients.  Though when I get a call later or the next day or an email from the listing agent, I will respond.

I would. Down here in North Carolina, appointments are secured by a third-party company, and the company sends a feedback form to each person who shows a property after the showing. Unfortunately, less than half of agents actually fill these out.

I agree that feedback is a GREAT tool to help you and your clients move a listing.  Personally, I try my best to offer any commentary that I think might help Sellers.

I understand the core of your idea, but wonder about the effectiveness of it. Would participants have to register? Or Sign in with an existing social media account? If not, how would you screen out any prankster responses?
